Here’s some information about how to prepare for the meeting, most notably, completing the Confidential Client Questionnaire. By providing us with basic information on the Confidential Client Questionnaire at least 2 hours before our session, we are able to review your responses and documents and prepare for the meeting, which means you get more out of the meeting.

Step 1: Gather Documents

Before starting your Confidential Client Questionnaire, it may be helpful to gather the following documents first. We prefer documents in a PDF format uploaded with the questionnaire, but you can send photographs of the documents using your cell phone and text the photographs to 904-398-6100, or you drop off copies at our Jacksonville office as well.

Required documents:

  • Photo of your driver’s license

Highly recommended but not required:

(DON’T STRESS about providing the documents below – we often have productive discussions without any documents. Don’t postpone our meeting to gather the below documents)

  • Copies of any existing previous estate planning documents, including wills, trusts, powers of attorney, health care directives, etc.
  • Copies of the most recent financial or bank account statements evidencing your ownership of checking and bank accounts, investment accounts, retirement accounts, and annuities. We just need the most recent month’s statement you have.
  • Copies of any stock or bond certificates. This usually relates to stocks not in a brokerage account or if you are a small business owner.
  • Any Premarital or Marital Agreement? 
  • Any Life Insurance or Long-Term Care Policies? 
  • Is there a Divorce Decree or Property Settlement Agreement for divorce under which continued obligations exist (child or spousal support, maintain life insurance policy, etc.)? 

If you have a digital or pdf copy of these documents, you can upload them with your questionnaire. You can send photographs of the documents using your cell phone and text the photographs to 904-398-6100. Otherwise, you can drop off a copy to our office during regular business hours.

Step 2: Complete Online Questionnaire

Next, you are ready to complete your Confidential Client Questionnaire. This form is secure and encrypted.

Some tips:

  1. Blanks are ok! Don’t know it? Leave it blank.
  2. Don’t stress. Just do the best you can. If all you can finish is your name and family information, that’s ok. We’ll go over the essential information during the Family Wealth Planning Session.
  3. I repeat don’t stress. For the Initial Consultation, we DO NOT NEED social security numbers, account numbers, or exact balances to the penny. Balances to the nearest ten thousand still give us a productive meeting.

After the Initial Consultation, the attorney and team will give you the exact datapoints we will need to draft documents.
